REGISTRATION

Register for Generate Camp.

1st PAYMENT
(DEPOSIT)

Pay a deposit for Generate Camp. Your deposit should be at least $50.

2nd PAYMENT

Make an additional payment for Generate Camp (at least $75).

3rd PAYMENT
(FINAL BALANCE)

Pay the final balance for Generate Camp. Contact Lance King for the final balance.

SPECIAL NEEDS FORM

If you have a special need such as a dietary restriction (sugar, gluten, dairy) or a physical restriction (wheelchair, no stairs, etc.), please print and fill out this form.

WAIVER & RELEASE FORM

This is a mandatory form which must be notarized. We will have a notary at the church on a couple days after morning worship.

WHO: Students who will have completed any grade from 6-12 by June 2026

(If the participant is under 18 years of age, the Emergency Contact must be a parent or guardian.)

MEDICAL INFORMATION

We include this section because participants will be away from home and may need medical assistance. Providing appropriate information may help in an emergency situation and will help us keep participants on schedule for medicines or whatever. Please know that we are not being nosy, but we care about each participant and want them to be cared for appropriately.


WAIVER AND RELEASE

NOTE: At the end of this Waiver and Release, there will be a place to sign for 1) the parent or guardian of a participant who is under 18 years of age OR 2) a participant who is 18 years of age or older.
